We understand that doctor’s visits can create stress and anxiety, but we’re here to help you in any way we can. Let us know how we can make your patient experience a good one.
In order to provide you with the best treatment possible, we need access to certain medical information. Prior to your appointment, please confirm that your doctor sends us your records or provides you with copies to bring to your first appointment. Also, please make sure that any referral or pre-authorization requests are completed by your doctor prior to your appointment.
Prior to your appointment, please be prepared to complete a health history form, which will include questions about your medical and surgical history, and your family health history.
