In order to provide you with the best treatment possible, we need access to certain medical information. Prior to your appointment, please confirm that your doctor sends us your records or provides you with copies to bring to your first appointment. Also, please make sure that any referral or pre-authorization requests are completed by your doctor prior to your appointment.
